Support at Home Care That Respects Your Choices, Culture and Independence

Inclusive Home Care supports older people to live safely and independently at home. We work with you, your family, carers and representatives to plan care that reflects your needs, goals, culture, language, routines and preferences.

If you're aged 65+, we offer an extensive range of home care services including personal care, domestic assistance, cleaning, respite, transport, meal preparation, home maintenance, nursing and allied health support.

We provide care with respect, kindness and clear communication. Our aim is to support your independence while making sure your care is safe, reliable and centred on you.

What is Support at Home?

Support at Home is the Australian Government aged care program that helps older people receive support at home. It replaced Home Care Packages and Short-Term Restorative Care on 1 November 2025.

The program is designed to give older people stronger rights, clearer information and more involvement in decisions about their care.

How Inclusive Home Care Works With You

You will continue to have an approved provider to coordinate your care and manage your budget. At Inclusive Home Care, this support is provided through a collaborative relationship. We partner with you so you can understand your options, make informed decisions and have greater control over your services.

Some people may want to organise more of their own care and services. Others may want more help from their Care Partner. We will work with you at the level of support that suits your needs and preferences.

Support at Home

Support at Home Service Categories

Support at Home services are grouped into service categories to help older people receive the right support for their individual needs, goals and level of independence.

Clinical Care

Specialised services to maintain or regain functional and/or cognitive capabilities, including nursing, allied health, nutrition, care management and restorative care management.

Independence

Support to manage activities of daily living and maintain independence through personal care, social support, community engagement, respite, transport, assistive technology and home modifications.

Everyday Living

Support to keep your home in a liveable state through domestic assistance, shopping support, home maintenance, light gardening, home repairs and meal preparation or delivery.

Support at Home

Short-Term Support Pathways

From 1 October 2026, the Australian Government will fully fund personal care services. This means you can access personal care services at no out-of-pocket cost if you’ve been approved for the personal care service type such as showering, continence support, dressing, eating or assistance with self-administration of medication in your support plan.

1. Restorative Care Pathway

The Restorative Care pathway is designed to support participants in regaining strength, independence and functional capacity following an injury or illness through targeted allied health services for up to 12 weeks.

2. End-of-Life Pathway

The End-of-Life pathway supports participants who have been diagnosed with three months or less to live and wish to remain at home by increasing the level of services available.

3. Assistive Technology and Home Modifications

All Support at Home participants have access to the Assistive Technology and Home Modifications (AT-HM) funding pool, which can cover:

FUNDING INFORMATION

Understanding Your Support at Home Funding

You can choose to pay for your services privately, or you may be eligible for government funding through the Support at Home program.

If you have been approved for a Support at Home package, your Notice of Decision letter from My Aged Care will show your assigned classification level, from Level 1 to Level 8. This letter will also outline the quarterly budget the Australian Government will contribute towards your care and support services.

The funding budget for Assistive Technology and Home Modifications is separate from the main Support at Home funding. Funding allocated for AT-HM may range from $500 to $15,000 to help make your home safer, more accessible, and better suited to your care needs.

If you are not eligible for government funding or prefer private care, you can access our services directly with flexible payment options.
